Actually I have a build lvl 87 (near 88) with :
I buy a lot of items, but it's cheap at this time.
Just boots cost 40c, Blue Nightmare 40c, Watcher 30c.
For a large cluster jewel = 1 ex and coward = 1ex.
I dont serch for amulet, but the annoint is expensive (2 golden)

2) If you don't have the watcher's eye/coward belt combo, just wear a regular rare belt.

3) No I do not think that is a good option. You will need Herald of Ash, especially if you don't have the Coward Belt yet. Play with a rare belt until you can afford the Cowards' Belt and leave Herald of Ash in place.

Replace Elemental Weakness in boots with Combustion
Replace Combustion in helmet with Cold Snap

edit : I buy a cluster jewel with 5 passives, with chaos res and 5 dex on small nodes. 4 or 5 passives change anything, we can skip the small passives with a 5.
