Still have this issue for most of the EA now. Today it's especially bad again, stuttering all the fucking time. Even gameplay freezing entirely sometimes for a few seconds. All with only latency spiking.
Edit: Just found that it's in fact not the game or GGG servers. There's some problem with (my) PC/Windows that causes frequent latency spikes across the board. They are short and other programs and games seem to deal with them in a way that I don't notice.
When I run pings for the PoE server and one of my servers there's a spike at roughly the same time, all the time. This does not happen on a laptop I put right beside my PC with the same pings running. There is's very consistent at ~20ms for both. It's pretty safe to say that it's not my Internet or my WLAN. No clue what the hell causes it.

For others with the problem I'd suggest to test this too. Run a CMD window and do
ping -t ANY_IP_OF_WHATEVER_SERVER_YOU_KNOW
You can do e.g. ping -t 1.1.1.1, does the trick for me too and has the same spikes at the same time.
Edit 2: Yeah, I can conclude that it must be this PC or its wireless adapter causing it. Other devices in the same WLAN do not show this behavior. Only this one PC. Sadly I currently can't connect with a cable so I can't check if that would fix it. Anyway, I strongly suggest others with this very problem pattern (regular ping spikes and stutter) to check for this too, it may very well be your PC, wireless connection or a combination of that.
Edit 3: I can play pretty much fine using a hotspot from my phone. Being mobile internet it's rather unstable in terms of latency which causes minor stutter in gameplay, but the huge spikes and heavy stuttering does not happen at all. It's 100% not the game causing the problem for me, it's just the only game that makes it obvious there is a problem.
Thanks for posting this information. I gave your suggestions a try and got some different results.
I tried dual pinging the PoE server and a server of my own and didn't get any spikes at all on either side in the command prompt, but did in game.
I then tried using a wifi connection vs. my hardwired connection, but still had the issues in game but not in the command prompt.
Booted and connected on my Steam Deck and DID NOT have the lag issues.

Try connecting directly to your modem/router with an Ethernet cable. Night and day difference vs wireless!! No more lagging!!! Just saw someone else post this and didn’t think it would make a difference so I tried it…..man was I wrong. Hope this helps.

First 2 days of EA were fine, no lag spikes. But every day after that I've been having lag spikes every 10-15 seconds.

I can only play with wifi (ps5), so... Is there a possibility that for those who play with wifi the excessive lag will disappear sooner or later or not? And I repeat that in the first days of early access I did not have the lag problems that I, like many other players, have had in the last few days.

i think this is the same problem they have with poe 1, there is no fix, i tried everything, i changed router, pc and internet provider over the years, same thing. sometimes you are lucky and you can load a map on a good server, but half of the time is unplayable. a 5000ms lag spike ended my sanctum run... i don't know if i did not notice the first week because of the hype or is something that just started now. i hope they fix it somehow.

I specifically pinged server which you pinged and another server which exist in Frankfurt, but not a game server. I got similar results where sometimes ping can jump from 64ms to 90ms.

Ok, I agree that maybe it is issue with provider that not all packages sent with the same speed, but the question is why this cause stutters in the game? There is no any other single game which force you to have ultra stable connection which send all packages with the same speed. I am getting stutters on closest server even when delay jump from 23ms to 34ms. I shouldn't be able to feel such small delay in the game at all, but I clearly see stutters. Also, it is better to compare game server ping and some other stable server from the same city, because increased ping to game server isn't 100% your network issue. Those delays could happen because of limited bandwidth on server if it fully loaded with players.
