Not so good at Photoshop but here's my submission:

Front of shirt, put an unidentified unique 'Heavy Belt' item text. Orange Fill, Black border, black text, like how Neversink does in filter. Like how unid unique drops on the ground.
Nothing else, no item picture, nothing. The pale flame of his heart disappeared in his azure reflection.
