Farrow wouldn't spawn in Mystic Refuge at all for me even after clearing the entire area. After Hotfix 11, I went to Mystic Refuge, cleared the entire area, and was able to summon Farrow. Once he picks up the book from the ground and starts reading, interact with him and play the dialogue for one of the volumes. I played the dialogue for Volume 4 and did not skip anything. Once the dialogue was finished, I was able to interact with the book in front of the Scrying pool. Hope it works for you as well! Yeah that didn't work at all.
