Hammer of Kamasa does not drop from Act 3 The Molten Vault Boss

First kill and drop but I opened the teleport because my inventory was full, I come back and the hammer is gone....

just killed him on the 3rd char (sorceress) and he didn't drop the hammer. probably a conflict somewhere because I finished this quest on my 1st char(monk) and 2nd char(warrior) without issues
