[POE 2] Bugs

The past 3 breaches I've attempted today all ended with the same crash.

1. I progress through a breach for 30-60s
2. Game freezes
- The UI is still responsive, and I'm able to go the pause menu and exit the game
- The game isn't fully frozen--the wind dancer animation continues to move, etc.
- My character and the enemy just appear stuck without attacking
- If I don't quit, then I'll eventually be kicked to the main menu with an unknown error
3. I load back in and my map portals are closed.
- I already completed the maps in all 3 cases, I left the breach for last
- All items I got from the Breach were lost, and it looks like it saved my items up until I entered the breach

Same map - 2 rare bosses spawn outside of the map.

just opened "singularity" strongbox, text reads "contains an additional unique item." no unique item dropped :<
cannot add quality to sorcery ward despite it saying it can
upgrades radius on time lost jewels does not make the radius larger. Despite having the prefix, the radius remains small
Bug Report Experienced on 31/12/2024 5:14pm GMT +8 Singapore Server

Description:
While doing Trial of Sekhemas on 4th floor received an affliction where "You have no energy shield" also known as Shattered Shield, my honor instantaneously dropped to 0 and failed the trial while I had 4000 energy prior and 3000+ honour left.

Suspected that the game registered my loss of energy shield as damage taken and deducted from my honour and failed my attempt.

Recreate:
While having energy shield, get the affliction named "Shattered Shield", the loss amount of energy shield should registered as damage taken and deduct from your honour.

Bugs experienced:
As of 31/12/2024:
- Cluster Grenade stops exploding further grenades after weapon switch
- Cluster Grenade sometimes randomly gets stuck on cooldown and is unusable until re-equipping the gem
- Glacial Bolt ice walls often don't deploy properly if fired at close range enemies (even if there is enough space and no big monsters in the way)
- Fragmentation Rounds feel like not working properly if fired at very close range enemies, which gets worse with small enemies (might be a pierce/fork related issue)
Unable to traverse to the End Game Burning Monolith for the Pinnacle of Flame Quest. Surrounding area is not consumed by fog of war.
Not sure if it's a bug or intended. Flask effect duration doesn't increase Olroth's resolve's heal.
The expanding red circle became invisible after I left the area and came back. I thought the circle already exploded, but got hit when I see nothing on the ground. Not sure what triggered this as I've left the circle ouside of the view and came back to it several times before, and this is the first time it became invisible.
