Have you tried deleting shader cache from appdata/roaming/path of exile?, set the game to preferred dx first, close it and then delete it, relaunch the game and wait for cpu usage to settle at login screen.

On a side note how high is cpu usage during those stutters?.

Ive deleted entire Path of exile folder many times, also doing that every league, and theres no impact on cpu stutters. Before shaders compiled freezes happens much more often, but it doesnt matter. Stutters happens on login screen aswell as in hideout or any other location. Most interesting, that situation variets for each launch.

I have to either use IDLEDISABLE power plan OR restart the game until it starts without stuttering (4-8 times usually)
