First, you took damage nodes before life nodes. Then you could at least link your boots and have your cast when damage taken setup in place.

You also dont have curse setup, or warchief/proctector.
Remove Ruthless, doesnt work with Cyclone, put fortify instead.

Help yourself and take a look at the guide again, specially the "Skill gems" and "Levelling Trees and such" sections.

In the gears section the Axe has ruthless support in it so I put it in too lol. I copied the 47 and then 60 leveling trees soo...

You need to try and replace your Tabula. Since you only need a 5L for the totem, and even then, I'd argue a 4L is just fine as well (dropping faster casting), you should be able to pick up a belly of the beast for really cheap. That said, you'd also need to re-color your weapon to try and get that to atleast a 5 or 6L for cyclone.

-No CWDT + Immortal Call setup
-No curses
-Missing Jewel Sockets
-Very low life rolls on both rings and boots, none on amulet, semi-low roll on belt.

Ugh, got distracted and forgot to submit. Posting this anyways.

It is exactly as you said, low HP. And with VP, your leech rate depends on HP. But here's a bunch of other stuff:

Re-do your skill gems. Ruthless does not work with Cyclone. You're using Herald of Ice instead of Blasphemy Frostbite. No CWDT + IC set-up. Duration does not work with Fortify. Use BM or something else. If you don't use Fortify belt, make sure to keep Fortify up when things won't die quickly.
Buy some better jewels and pick-up another jewel near Golem's Blood below Marauder tree. You have no jewel in top right, but if you put a Fertile Mind, that will relieve your INT requirements and be able to swap a lot of gear around easily. You also have Ring of Blades for some reason? If you switch to all jewels with 35ish HP and 2 dmg mods or missing resists, that is about 80 HP (200ish HP) plus extra dmg or w/e resists.
Grab that extra Frenzy near Golem's Blood as well.
More HP and movement speed on boots. Another 100ish HP.
Belt can get another 30 HP, so another 100 HP.
More HP on rings, another 100-200 HP.
By switching to Fertile Mind, you can get an amulet with 70ish HP, so another 200 HP.
Switching to 5L Carcass Jack will be similar damage, but better HP and range. Another 150-200 HP.
As you get more levels, HP will jump up a lot. Level-up HP, Constitution, more jewels, more HP %, etc. I think most people around level 90 are sitting around 4.5-6k+ depending on how safe they want to play.

Flasks:
You shouldn't need mana flask anymore. Especially once you switch to 5L and get a few more levels. Enlighten level 2+ will also help with aura mana reservation.
You need to have a flask to remove freeze and a flask to remove bleed. Once you get Avatar of Veil, you just need to have bleed and can optionally get remove curse. If you get the bleed on movement and Cyclone around, your HP will drop rapidly and possibly faster than your leech.
Better roll on quicksilver will go a long way. I like the extra movement speed, but several can do immunity as well.
Can replace mana and 1 HP flask with cheap stuff like Sulphur for more dmg or defensive flasks.

Heading out shortly but will comment more later.. Till then just from passive tree alone you are missing 40% maximum life, 1 frenzy and spent 3 points to get 1 jewel slot instead of 2 points near Golem's Blood vs the 92pt kondo tree listed on main guide.

Changing just those would give you more life and the extra frenzy goes a long way. Try making those changes, as well as some of the ones people have mentioned. Should be fine.

Alright have a moment now so lets go over this.

Link your Devoto's, Hrimsorrow and Kondo. I would drop the tabula for even a cheap chest with some life on it. A Cheap Carcass Jack even better.

Drop damage nodes to follow the passive tree. Just take Vaal Pact if you intend on using it.

These changes alone will help you start surviving more. I will relink the Shaper kill gear with the 3.1 setup. Didn't think anyone would even look at those links but here we are! ;)

The other posts guys have posted before have the other things you can work towards. This is just the basics so you can start clearing maps comfortably while making the other changes.

I'm not the fastest currency generator either and I burn a lot of it just trying out new things.(Elder weapon crafting anyone? Sadface.) Ya get better and better at it the more time you put into it.

I think total currency I have used so far in league for the gear I am clearing t15s with(no t16 maps dropped and no way I'm buying them!) is around 3 Exalts-ish. I am not really going for a disfavour in league which would be the main big currency item. Since I have two 6l disfavours from last league in standard and the elder weapons are just so interesting I've been playing around with those more than trying to stack currency this league.

You'll slowly start to find/buy upgrades and once you get the last ascendancy clearing maps will start to feel really good. Just remember the higher your HP, the bigger your leeching is and the more you'll survive.

Passive tree looks good now, minus that one extra jewel but meh that's fine. One thing I would definitely pick up as soon as you can afford it is Fertile Mind jewel. That way you won't have to worry about Int on gear and can level up Frostbite all the way.

I don’t mind answering questions if I am on but I do check this thread quite often myself as do others. Your question may also help someone else who is wondering the same thing if it is asked here.

Don’t feel like you are “spamming” or anything like that. To be cliche… “The only stupid question is the one you don’t ask.” I don’t know how others do it but if I didn’t want to answer questions I would not have made the guide. :D
