WTB - Top-tier minion support Ghastly Eyes

Howdy,

Looking for 2-3 Ghastly Eyes with the following affixes

1 - Tier 1 Life (46-50)(will also consider Tier 2 life for perfect damage-affix jewels)
2 - Tier 1 added physical damage to minion attacks ([15-17]-[22-26])
3 - (15%-20%) increased minion damage if you've used a minion skill recently
4 - One of the following, in order of preference:

* (4%-6%) Minion attack and (4%-6%) minion cast speed (vastly preferred and my main goal)
* Other useful suffixes (all-element resist, double-element resists, increased cast/attack speed if you've killed recently)
* Open Suffix for slam attempts
