If you use kaoms and tombfist, you will not have enough slot for you gem. You need two items with 4 slots.
If you can manage without a cast on damage taken, it could work. But I prefer a rare boots, to adjust my resist (and get chaos resist on it) | |

Why do you take outmatch and outlast in your HC shield version? Doesn't that lose half its functionality since you don't have an off hand weapon?
