Ritual quest chain bugged

After killing the Ritual Boss, it is no longer possible to attach the Head of the King. The quest chain is broken – in the Ritual Sanctuary, you are asked to attach the Head of the King, but nothing happens – it is impossible to progress through the Ritual Atlas.

Same issue here. Clicking the effigy with the head of the king opens up the atlas map, but doesn't allow me to select 5 maps to continue the quest chain.

Same, the bosses icon is still there in the arena and the map is blue despite having the head and asking me to unlock 5 maps

same issue, also causes king of the mists to be infinitely repeatable without requiring any new invitation.
