invisible and invulnerable breach monsters

I've ran into this issue at least twice now. I can get mouse over the nameplate but not see the monsters. The monsters can't be attacked in any way but they can still attack you.

Just got this one too. also had the "chance to fracture" mod, although not sure if that's the issue

Unfortunatly died to this bug, can confirm unstable breach and fracture Rare mod on orginater map!
