One other very real possibility is that you just stood on top of an on-death effect, which didn't get telegraphed at all.
Those do incredible amounts of damage and you can easily have 3+ of those stacked on top of each other without you even knowing. |

Something involving Bleeding is also another strong possibility .

Chaos dmg doesn't bypass ES. I think it did in PoE1 but in PoE2 it just deals double damage to ES.
