Equipping an item deleting the item in the previous spot

I had a crazy good wand worth 30+d disappear and be deleted from accidentally equipping a white wand trying to id an item next to it.

I'm on console I have video proof.

I had room In my inventory for my old wand to go. I checked the entire ground in the map. Picking up any items in the area and moving them to be sure it wasn't hidden.


I crafted this wand myself and was super excited to have actually self crafted such a huge upgrade. It over doubled my dps.

This happened again. In a 10 breach map.

Is there any response on this? This is a game breaking bug. Having items delete not go to ground or inventory is absolutely an issue.
