i had this happen to me just a few minutes ago, i sympathise to how fucking retarded that shit is. and i agree its bad design, a lot of shit in this game is badly designed and thats why we post this here so they take it out of the game so it becomes a better game. cause right now it sucks hard.
Ye terrible mechanic, they should add a feature so if your standing on fire and not watching your monitor you wont take damage from it
As a hardcore only player, I will say 2 things:

1. In POE2, you should never ever stand near any corpse unless you are 100% sure the map is safe.

2. It is inconsistent. Some examples: Jiquani's big robot will cancel its attacks when it dies. the Sekhema Trial floor 2 fire & Ice twins will make huge explosions when they die, but these are cosmetic only and do no damage. However, many, many rare monsters will leave an explosive bomb on the floor when they die which will kill you.

So, GGG should make the game consistent in whether things are or are not dangerous after they die. With some that are and some that are not, you have to assume everything is dangerous.
Hardcore
I'm considering taking a break until they fix some of this poor design. On death effects are a cheap shot and just feel bad. Quite often, I roll/evade and still die because the effect is too large or I can't see it at all due to the noise on the screen. The problem is made worse playing melee.

Some fixes:

1. Remove on death effects from the game.
2. Provide more portals in maps, so you can still get loot.
3. Make the on death effects more avoidable or not one shot events.

I have seen many "get good" comments in this thread. "Get good" does not defend poor game design, nor does it make the game more fun.
