damn yup just ran into this bug also
was trying to run one of the unique relics trying to enter third floor and out of sync error |

You can Ctrl + click the portal and hit create new instance and it should fix it, that or cancel and do a new run.

so far I've gotten this bug 4 times, every single time right before going into the trial of time. I can't afford to run any unique relics if this keeps on happening lol.The only solution for me is to restart, thus wasting a key.
