condition to fail a map

Hi, i still dont totally understand how the map system work

if i die in a map, it kick me out and i can retry it with a new waystone..
what are the condition to fail one ?

the only one i managed to fail was a merchant one, i just couldn't get it completed, and i used all the 6 portals, and now it say's failed and i can't progress thaat direction anymore.. so nice..
