Everything seems fine except a few things. I would drop a second life flask and will use something else.
Also i hope you don't run temp chains along with witchfire :)
Since you have 15% chance to poison on quiver be sure to drop this chance on the tree and possibly take another jewel or just more attack speed.
On Blood Rage.. i won't use it for Uber Elder i guess. Degen would be too much for you.

Yeah i know for life flask i must to try something else, what flask should i use, maybe dodge? If i using Blood Rage i didn't get degen, my life always up.

Isn't % damage leech better for this build ? or leech don't work with chaos DOT ?
