For shield, I think spell block + mana recovery is better since I often run out of mana with MoM and any additional recovery source is very good. Life gain on block from Bone Offering is good enough but additional recovery on shield is okay too.
For me, having around 400 life more is better than the recovery. Life recovery from bone offering is more than enough. With Lionseye I have around 7k live and 3k mana + 1.5k ES. And Spell block on the Amulet is better because you can have decent amounts of mana + spell block and life on it. Shields with spell block have much less.

Pretty easy, especially if you're willing to level your own gems.
You're going to need just over 50 regrets, that's ~30c.
5L Vis Mortis is going to run you another ~35c.
Bones of Ullr, Skullhead, Advancing Fortress, Lioneye's Remorse, Lazhwar's Amulet are all going to cost you basically nothing. 5c total, max.
New gloves without fire damage, 4-5c.
New rings with life / mana / resists are probably going to be one of your more expensive items. If you get one that's good enough, you can keep the essense worm on your other hand and run haste or determination.
Your ascendancies are going to need to be shuffled too - drop spirit eater and beacon of corruption for Soul Weaver.
With your budget, you can probably pick up The Scourge to boost your damage, perhaps even a rare helm instead of Skullhead, or a rare shield instead of Lioneyes - one of the new ones with %block chance applied to spells is nice, lets you drop Lazhwar's and put in whatever amulet you like. If you go with the Scourge claw, drop the Vinktar's for a Rumi's.
does flamability curse or elemental weakness interact with spectres damage? i mean, as someone who doesnt play hardcore i could just go for extra damage from flamability instead of the more defensive curses suggested by the author.
