Sure thing. My characters should be public, name is "MyHarbingerFrostBlades". I haven't invested in gear so far either (all self found / crafted so far except for the cheap 5L chest). I am level 81 and have taken all the life, life+eva nodes I can reach for now so that's probably where the difference is.

I'm using acro right now with and plus a total of 9 frenzy charges. Full buffs I'm getting a 66% chance to evade attacks and also a 50 something chance to dodge them.

I'm lvl 69 and struggling with str and int requirements, where do you guys get your str and int from?

You'll definitely want to start with getting more life. Just my belt alone gets me almost 400 hp with belly. I'm up to map tier 12 with the following setup and 4750 hp:

Another question about the Curse on hit setups.

You have two curse on hit, one with assassin's mark, the other with Frostbite. But you actually haven't any passive or gear allowing you to cast two curses at the same time.

Is it worth to use two 4L configuration that are going to proc one after another instead of something else like a golem ?

Or maybe I am missing something and your two curses are procing simultaneously.
