After finishing Act 3 and entering Act 4, I couldn't get the quest item when I spoke to Alva.

When I first went to Kingsmarch, I couldn't get the quest item from Alva, and now I can't find Alva. Because I can't find her, I can't progress. I've spent so much time and used up divs for ASD points. Please help!
