Omen Of Amelioration does not work

Omen of amelioration was not used when I had it in my inventory, don't know conditions of this happening but:

I was in mirage harvest tier 16 rare + corrupt map
At the time I died, i had like 600ms ping latency (could be this possibly)

No text showed in the chat, just that I died
