.dll and 0xc0000005 crashing the game

Faulting module name:
PathOfExileSteam.exe
ntdll.dll
nvwgf2umx.dll
nvgpucomp64.dll
KERNELBASE.dll
dxcompiler.dll
fmodstudio.dll

all under Faulting application name: PathOfExileSteam.exe
all with the same code Exception code: 0xc0000005

that exception code 0xc0000005 generally means memory error, however iv done memtest / windows memory diagnostic and my ram sticks are fine and no errors were found.
so far iv done:
uninstall steam / poe / validate files
DDU Uninstall gpu drivers while in safe mode "internet cable unplugged" / clean reinstall driver only (not nvidia app)
clear shader cache / delete all file contents
clear nvidia shaders
nvidia shaders file set to unlimited
DISM /Online /Cleanup-Image /CheckHealth
DISM /Online /Cleanup-Image /ScanHealth
DISM /Online /Cleanup-Image /RestoreHealth
sfc /scannow
download new dotnet sdk
download directx end-user runtime
allow the poe to be on the trust list in windows defender
various setting combos vulkan / dx12 upscale modes etc etc..
clean boot
run poe with nothing else running

iv even ran Furmark for an hour and no problems...

im sick to death with poe crashing soo much. i love content like ritual / ultimatum but more often than not i crash in the middle of a mechanic potentially wasting a map / scarab set

FIX THE GAME GGG !

Win 11
AMD Ryzen 5 7500F
Nvidia GeForce RTX 5060 Ti
32Gb DDR5 RAM

i can post any of the logs / dumps if needed, im getting really desperate now, my pc is only 4 months old

0xc0000005 is a pretty generic error code, tbh; I wouldn't rush to associate it with RAM.

The first thing I'd try that you don't seem to have already attempted is reinstalling the game.

I would also be curious whether performing a Clean Boot and just running the game without opening any unrelated applications (e.g. Discord) helps. I do not like the list of DLL files being implicated in the crashes - it suggests there's no rhyme or reason to the instability. While Kernelbase is often blamed inaccurately, seeing Nvidia software, Windows files, and even FMOD appearing is concerning.

oh yer sorry i forgot to add that to the list i also did the verify integrity of game files, uninstall poe / steam and reinstall

those dlls iv mentioned a friend said its mostly shaders related... i have no idea tbh

im currently in the clean boot and just done 2 maps with no crash, its currently 11pm for me so il do more testing tomorrow TY SO MUCH for the help

this is getting soo annoying, iv never had soo many crashes
Need to +1 this because the game is unplayable for me at this point. It's not just 0x5c, there are multiple for me:

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Exception code: 0xc0000005

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: dxcompiler.dll, version: 1.8.2407.7, time stamp: 0x66a9ae4f
Exception code: 0xc0000005

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: ntdll.dll, version: 10.0.26100.8115, time stamp: 0xa571fe98
Exception code: 0xc0000374

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: nvwgf2umx.dll, version: 32.0.15.9597, time stamp: 0x69b98c74
Exception code: 0xc0000409

I have:

1) Reinstalled Path of Exile three times, once to a different hard drive altogether, no difference.

2) Verified integrity of files, no errors found.

3) Patched Windows (currently 25H2 OS Build 26200.8117)

4) Updated drivers from motherboard manufacturer to the latest (MSI PRO Z790-A MAX)

5) Flashed BIOS to latest version (16.1.40.2765)

6) Reinstalled graphics drivers, clean installation, to current version (4070 RTX, version 595.97)

7) Reverted graphics drivers to 581.42, clean installation

8) Reinstalled operating system files (kept current files, just reinstall OS files)

9) Ran Windows Memory Diagnostic, no issues found

10) Ran Chkdsk, no issues found

11) Deleted my shaders cache (this exacerbated the issue)

12) Changed all settings to the lowest possible, including disabling hyperthreading (this helped me not crash the second I tried to load into my hideout but the crashing persists).

All of this began happening after the latest patch and the game is currently unplayable for me - crashes will happen multiple times per map, during Maven Invitations, or even just sitting idle (go to use the restroom, come back to desktop). I can't even load into Kingsmarch anymore without an immediate crash.

No other game does this - I've tried Microsoft Flight Simulator, Battlefield 5, Nioh 3, Rocket League, some others, and no other game is crashing, only Path of Exile. The latest patch seems to have broken something and I can't play anymore.

Kindly asking GGG to look into this because I'd like to consider myself to be very tech savvy (I work in IT....) and I'm out of options at this point. I'll be more than happy to provide additional diagnostic info if needed.

Edit: Since I wanted to try this anyway, I did a full, 100% reformat of my hard drive and did a clean reinstall of Windows 11. I've only downloaded Steam, Path of Exile, and the drivers I mentioned above. The minute I opened up Path of Exile I crashed again with the same error logs:

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: nvwgf2umx.dll, version: 32.0.15.9597, time stamp: 0x69b98c74
Exception code: 0xc0000409

Faulting application name: PathOfExileSteam.exe, version: 0.0.0.0, time stamp: 0x69c5d12b
Faulting module name: dxcompiler.dll, version: 1.8.2407.7, time stamp: 0x66a9ae4f
Exception code: 0xc0000005

Again, this is on a brand new, fresh installation of Windows 11 with nothing installed except Steam and Path of Exile. I don't think there's anything left I can do on my end...

Same, did full reinstall of game, windows, reformat, windows on other m.2, stress tested every single hardware, everything is fine, poe is not. Same crashes as mentioned here, i7 13700k on z790 tomahawk, arc b580 all latest drivers and bios, start happening with last 2 poe "hotfixes". It makes no sense to me why poe tries to call for igpu dll when it has other one selected in game (even disabled in device manager). Had same issue in keepers when league mechanic was on screen and was using vulkan. Now its happening in vulkan and dx. Also seems to be have no triggers, can happen afk in ho, or in juiced map.
I've now even tried uninstalling every Windows patch (since the March causing C:\ to be unreadable problems...) and also tried pulling out one stick of RAM at a time to see if there was an issue there but still to no avail. This seems to happen mostly when I'm trying to fight Saresh or loading into a Mirage more often than not, but I'll still crash when loading into town or killing mobs in a regular map too. I have a strong feeling there's some issues with new items / mobs / textures that aren't in the cache that causes the crashing because when I delete out my shader cache and have to "re-download" them, that's when the crashes become extremely frequent, but once they do cache, I'm able to at least complete a single map before it happens again.
Okay, well....

In the "stupid problems require stupid solutions" world we live in, I think I've managed to solve it...

All I did was go into the nVidia app, find Path of Exile, and let it "Optimize" the settings for me, with the one exception being "Full Screen Windowed" instead of "Full Screen" and I've played for 30 minutes with no crashes - the game even seems to run smoother.

If anyone here is still having this issue, I'd highly recommend doing the same, or check your graphics settings for the following (I'm running a 4080 RTX):

Render Resolution: NVIDIA DLAA
Renderer: DX12
VSync: Off
Triple-Buffering: Disabled

Antialiasing Quality: High
Upscale Mode: NVIDIA DLSS
Sharpness 15%
Lightning: Shadows + Global Illumination
Shadows + GI Quality: Ultra
Sun Shadow Quality: High
Number of Lights: High
Bloom: 100%
Depth of Field: Disabled
Water Detail Level: High
Texture Quality: High
Texture Filtering: 16x Anisotropic Filtering

Dynamic Culling: Disabled
Dynamic Resolution: Disabled
Engine Multithreading: Enabled

I can't tell you what the settings were at prior (I'm willing to guess those nVidia-specific settings might be important...), but now I'm finally able to play this game that I'm horribly addicted to.

Good luck everyone and I'll update this if the crashing starts again, but it's been buttery smooth for now.
