Mirage portal exit will not acticvate in Chamber of Sins Act 7

Xbox Series X. I have experienced this consistently a few times now. I am able to enter the Mirage but once i clear it and try to exit the Mirage exit will not activate. I am forced to quit the game to get out of the mirage. So far this has only happened in the Chamber of Sins in act 7, but it has happened every time in Chamber of Sins act 7.
