Incarnation of Dread Disconnect

Hello, playing in Phrecia 2.0 and trying to get my challenges done. Attempted maven witnessed incarnation of dread, when I killed the boss it popped up in the white text "maven witnessed complete" or whatever it says and then I got immediately disconnected. My favoured map didn't unlock, Maven didn't actually witness it, and all portals were gone with no loot. I did die once in the fight and came back, I saw some tip from months ago that said you have to not die once maybe. Bummer, and cheers.
