Maps disappearing from stash

I have lost quite a few maps when I have gone to put recently gained ones into my stash and all the previous ones disappear and only the most recently gained one remains. This has now happened several times after defeating getting both the Omniscient and Grasping Voidstones. I think it actually happened once before, but I wasn't sure. It has happened several times now.
