Legacy of the Rose Shaper Memory proxy gets stuck

I have encountered a recurring bug with Legacy of the Rose where the Shaper Memory proxy (in this case, the Shaper's Devastation version) gets stuck and does not despawn or take any further action after the skill is used. Because he is stuck in the world, the player is not able to gain further Remembrance stacks since the skill/item still recognizes the Shaper Memory as being summoned.
