Were you applying other curses without also increasing your curse limit?

I assume you were testing it on your Bog Shaman, who has Elemental Weakness linked with Cast When Stunned in her helmet, and at a brief glance, I don't see any "You can apply an additional Curse" effects like the Whispers of Doom passive.

Do you have a Path of Building or anything?

QA wouldn't be able to investigate this unless they had a decent amount of insight into the passives, equipment, and so on that you had been using at the time of the issue.
