Second Graft and Slot disappearing, many suggested fixes already tried (listed)

Am playing on PC and this is in Phrecia 2.0. I had both graft slots unlocked and 2 equipped, I made a second char in the league and now I no longer have a second slot for a graft and also the graft I had equipped is gone.

I tried manually replacing, clicking the arrow to expand/collapse to see if it was there, restarted, changed from using controller to using a mouse, looked in my stash and other char to see if the missing graft had appeared there for some reason, spoke with Alith, bought a graft from her, had the tree produce more grafts, ...

Funny enough I got skill points for feeding the tree but still the second slot and second graft didn't show back up.

None of this has corrected the issue.

Anyone else experiencing similar and have found a solution for this case?

-edit: Forgot to include that while out fighting the skill attached to the missing graft was not being used as I had seen in some other posts so it doesn't seem to just be hidden in some way but usable.

UPDATE: After going out and getting another of Head of Vruun and turning it in it opened both slots again. The graft that was eaten is still gone however.

Did you start in SSF Phrecia and Migrate to trade Phrecia?

If you did, did you Migrate All?
