King of the mist

During the third phase (second part when of fighting him), he transformed up on a platform and could not get back done. I've noticed he usually teleports right before transforming. I'm guessing something with his teleport allows him to sometimes pick a spot in there to teleport to. Hope this helps find the issue and fix it.
