Wait sixty seconds to zone in to a map only to discover you are already dead, rinse repeat till all progress on the next level has been completely UNDONE. Awesome, fantastic, super wonderful, 0/10 league.

i think this could be fixed if they didnt spawn breach on the starting portal. this scared me to death the first time i spawned on 3 rares.

Is this to do with map entrance spawning on top of a breech spawn? I noticed this a few times in SC and seems a bit sketchy. Don't remember anything like that being in the game before. Poe2 has some spawn points where you spawn on mobs as well. It's unusual, but particularly dangerous because you can spawn in without having HP and life replenished. I don't think its great, unless mobs have been kited there, which is fair enough and the player knows about it, rather than spawning into combat.

Perhaps this is related to the lore. The Atlas of Worlds is practically entered through the Atlas Device via a portal, like in Stargate. Now, we have successfully fought and defeated great evils in the Atlas of Worlds for many years and have advanced so far in the infinite vastness of space and time in the Atlas of Worlds that we now have new and more powerful evils, and even Uber Uber evils. intelligent, wise, and powerful that they can even station entire hordes at the entrances so that you cannot enter the Atlas of Worlds through a portal without spawning directly in the middle of the battle.

Yeah, it seems to be only on a very small amount of maps though. I only encountered it a couple of times and am pretty close to atlas completion, was pretty unexpected.
Really the game should have some kind of fail-safe preventing mobs from spawning on a start of a map, or at a door you would click like in Burial Chambers. That has happened way too many times to me and the only way to get out of it is get lucky 4Head
Ok, it happens once because you didn't see it coming, fair enough. But there is grace period and there is a look on the exp bar, telling you you don't want to lose all that in one spot...YOu don't go where you died over and over again. I learned that the hard way, too.

I died simulacrum wave 13 the other day with 60% on my exp bar. Obv i did not go back in there.

The problem is this will effect players in HC. For me it was not just that I spawned into mobs, but I spawned directly into the breach that opens automatically on proximity. I didn't die, but the game is previously programed to not work like this, and it doesn't seem to be intentionally designed to do that.

Blocking simulacrum exit is not the same thing. Simulacrum always begins away from the exit and is proc'd by interaction with NPC. Yes it is possible to brick encounters by dragging mobs onto the exit, or positioning portal in a bad place, but this is without warning. It would be possible to pause and identify risk before moving in the maps incase of the small chance of this spawn, but this has not been needed before. Also then what? Log out or escape from portal and right off the map because it is too dangerous to enter? That is not a good design to introduce. I think it is probably an oversite in the programing that is allowing it to build on the entry, and should be patched.

OP is also pointing out, that a reason for not having mobs on the spawn point, is if for some reason the spawn in, is delayed, like can happen with POE loading, spawning into a safe area will not result in character death.

I think in general, it sort of does up until now, i've never encountered it as being a problem. You might of identified one on a tight arena where it can be a problem, certainly that is a sketchy arena. Incursion sort of spawns into mobs, but not right on top of them and it's know that, that is how the mechanic works.

I've certainly encountered bricking the entrance by drawing too may mobs. It can happen pretty easily with some of the underground expeditions. Should not happen on the first entry where player is unaware. I agree with you that there should be something preventing that.
