"Mania" debuff from "You're the crazy one!" not working properly (I think)

I do not think the "mania" debuff from "You're the crazy one!" is working with triggered (cast when damage taken) spells. It also doesn't appear to be working with the damage taken from poisons.

I have been running simulacrum from level 98 to 100. After hitting 100 I switched bloodlines from Olroth to Delirious and chose the node "You're the crazy one!" thinking it would be fun to use with the Beacon of Madness boots. I noticed there was two "Embrace Madness" skill icons...I made sure that I was using the Embrace Madness from the ascendancy notable and not from the boots.

My damage did not change on the tougher monsters in any noticeable way. This is very unexpected because the tooltip says "4% increased damage taken...can be inflicted up to 15 times." I can definitely notice the reduced action speed, but again, no difference in damage even in the prolonged fights that would surely give the monster enough time to acquire 15 mania debuff stacks (60% increased damage taken).

I could possibly see how being attacked less-frequently because of the reduced action speed might counteract the increased damage taken, but even the individual cast-when-damage taken procs do not appear to be doing any increased damage.
