Catarina's bloodline ascendancy Death offering kills players unexpectadly

Setup:

The queen's hunger linked with More Duration Support + Efficacy + lifetap
Leash of oblation belt

A lof of increased duration.

Amulet anointed with hollow effigy

All offerings have 16.25 base duration.


Character:

Expected:

Offerings always up, character stays alive.

What happens:

Out of combat, or in hideout I live forever, while fighting mobs it randomly kills me.

I've ruled out:


Being silenced (I'm silence immune)
Stun (Using kaoms roots)
Cost (Using lifetap and insane life flask recovery, it happens way too often without me being getting to low hp to be cost)
Map mods (Running white maps)
Using cyclone, not using cyclone..

Ran out of options what is killing me?

Bonus:
Runegraft of the warp does not work with offerings, I understand that this could've happen but it feels weird when offerings are clearly buffs when coming from queens hunger or necro ascendancy

Do you always die after the first duration runs out, or is it random? Have you caught on to any other pattern?

I already removed this setup but I did do other testing:

When I had very high duration it didn't die, (~20 secs)
At 16.25 I died consistently in combat.
At 17ish seconds I would die less but still die.

I confirmed cost wasn't the cause, I switched to eldritch battery and I it was impossible not to be payed

I couldn't replicate it consistently, it seemed to just happen randomly in combat.

In hideout, or out-of-combat in the begining of maps, I would live forever.

The only one that lasted 2 maps without dying was having the duration at 20 seconds +.

But in theory if the duration is above 15 seconds it should work.

Even at 17 seconds deaths happen.

My theory at the time was either desync, or something causes the queens hunger to delay.

I don't think it's desync since in HO I could run forever. (In HO it doesn't kill you but it reduces your hp to 0)
