Softlock - Unable to equip graft given by Ailith in act 1

I just started my second character and upon speaking with and accepting the first fleshgraft from Ailith on the coast I am unable to equip it. I'm not sure if i'm missing something but the slot just isn't there for the fleshgraft to be put into.
