Nameless King/King in the Mists Bugged after Maze Phase - Cannot be Damaged/Does Not Transform

After completing phase 1 and doing the maze phase - upon returning to the arena the King in the Mists is stuck on the stairs - he said his transformation voice line, but does not transform to phase 2 and cannot be damaged.
