Gruesome grafts challenge doesn't always count correctly

The requirement is supposed do be item level >= 78 and 4 mods on the grafts.

I have had several grafts fail to count despite meeting those requirements.

One of the failed grafts succeeded later in my hideout, but so far I haven't been able to get more than one graft to register per area.

I have a replicating Eshgraft and a draining Eshgraft with this problem on them at the moment.

I would recommend including screenshots with advanced mods on of the grafts not counting. I did several last night throwing on random Grafts without changing instances (after I noticed it was a challenge).

Also, have you tried typing /recheck_achievements in-game?

I was deceived by some mods that had more than one line of description so they looked like 2 separate mods each.

Using an exalted implant fixed the items.
