Can't Equip Second Graft

I have unlocked the second graft slot, but whenever I try to equip a second graft, it just replaces the already equipped graft. This happens no matter what types of graft I try to equip (this is not an issue of trying to equip two grafts of the same type). This is on Xbox.

you have to take rhe second graft with "Y" and move it manual to the slot. dont press "A" for equip.

When did you obtain second graft slot? Did you obtain the boss head in maps or acts? What tier maps?

I got mine in my second Tier 1 map. With Hive Fortresses also starting to appear in maps from Tier 1, I wouldn't be surprised if this was also limited to maps.
But, I also only stablized one or two portals during acts, so I could be mistaken.
