Completed tier 13 map with 4 modifiers in a corrupted nexus atlas node.

Upon completion of Necropolis map zone boss, the map marked as completed and the zone on atlas became cleansed without any corrupted nexus boss spawn, and no subsequent atlas points were awareded--nor was the questline updated to move to the next tier (tier 4).

Note: I died on the boss encounter and had two attempts remaining. The second encounter was simple and over quickly.
