Omen of Amelioration - used all stacks on death

I just died in-game, in the Crypt map (due to a lag spike, but don't worry I'm not complaining about that!), and in my inventory I had 3 stacks of the "Omen of Amelioration" activated.

Well, when I died, it seemed to consume all 3 of the stacks. Wondering if this is working as intended (assuming not), is a bug (assuming so), and if anyone else has experienced this.
