I do not know if the patch 0.3.0 c is the cause of this bug, but jump completed Moulten Vault and the Hammer of Kamasa did not appear.

Same experience for me.
Pre-0.3.0c the Hammer of Kamasa dropped for me. Today, on another character doing the quest, it does not drop. Killed him twice to see if maybe I did something wrong but he doesn't want to drop the quest item at all. |

Same for me I even tried resetting the instance twice, quest item is not dropping for a new character.
