Breaches not closing

Tonight I did 8 breaches and I like to do them early in the map and then complete the map. I did the breach, killed everything in it but it never closed. The purple circle never receded back on itself and i didn't get breach splinters.

I thought maybe there was some mob hiding some where but maps have said 0 creatures left on the board. Even the Clasped hands stay - and when I say stay i mean Ill go and finish the map 10-20 min of time and come back and the breach still wont close.

This has to be a bug right? or am I missing something?
