Instance Refresh Randomly

I have a problem that is becoming more and more annoying, it is about exploration, every time I am in an instance, it resets randomly, causing the fog of war in areas that I had already explored to reappear on the minimap, making the gameplay slower, this has been happening to me continuously during the second season, sometimes during bosses, or when I am in the middle of a fight with mobs, please tell me if you have been able to solve this, or what it could be due to.
