character keeps committing suicide for no reason - PS5

my character keeps committing suicide, i can't cross half the map without it happening. it's not a flask issue or a side effect of a skill or gem, just keeps committing suicide out of the blue. sometimes it looks like i took damage but not kill me and all of a sudden dead, sometimes no one is near me and i just die and it'll say character committed suicide.

separate issue but maybe not, i keep having these audio glitches too

What are you playing? Are you dying to something like Volatility?

i am playing a chaos witch, not using anything that generates volatility, skill, gems or items. i just went back and checked to be sure but yeah no volatility nodes on the skill tree, no items or skills or gems that generate volatility.
it just started happening out of the blue a few days ago, that along with some audio issues. but the dying is the worst part. can't cross the map without 'committing suicide' even when i don't use skills at all and just try to sprint through the map. it wasn't doing any of that a few days ago, i'm on PS5 btw

i figured it out my bad guys
i must have been too tired or something but i got this unique on 'heartbound loop pearl ring' i think that's the issue it didn't register in my head but one of the mods is '300 physical damage taken on minion death' that's gotta be it, i just can't tank the damage yet lmao
