Died and dropped loot - Iron Citadel, Abyss, Headhunter

During the transition phase of the Count turning into a wolf, my headhunter buff with abyss ran out. I randomly exploded, died, and dropped several abyss omens from my corpse when I died.

I do not know why, but to lose a citadel to a bug like this is very unfortunate.
There were no enemies on the screen and I was just waiting for him to finish turning into a wolf.

Abyss modifiers seem really wonky with headhunters and target you as well, but this is the first time it happened like this so I suspect it might be a niche thing with the Iron Citadel boss.
