Inscribed Ultimatum disappearing act!

I was going through act 3 in Uzateel, I found the Altar in which the "Inscribed Ultimatum" sat. I grabbed it then immediately went to "Trials of Chaos" and complete it. I died during the boss fight normally no bugs. I clicked to respawn and go retry the Trial. When I went to the Trial Altar to try again the "Inscribed ultimatum" was completely gone from any chest, inventory, floor, and the altar itself. I reloaded the Uzateel Instance and it did not respawn the Ultimatum. I even re started the instance for Trials of Chaos and that did not fix the problem. PLEASE GIVE ME ANOTHER INSCRIBED ULTIMATUM.
