Can't progress simulacrum encounter

During a tier 1 simulacrum encounter after wave 2 or 3 the stash unit and next wave start totem did not appear. I can see the stash icon on the minimap (don't believe it is there during the combat portions of the encounter). Also the "# monsters remain" in the map information shows as zero. I have teleported out, logged out to character selection, and completely exited and reopened POE2; upon reentry the instance is still in the same state.

Update: After one of the instance reloads the path to the next area opened.
