Reforging Bench Missing

I have been playing on hardcore mode and died in ACT 4. Now I do not have Reforging Bench.
I have tried to migrate, but have nothing to migrate to:

I have aquired normal bench from ACT 1 but cant get reforging bench.



I have done quest Treasures Of Utzaal on hardcore and now boss of molten fortress does not redrop the item to acquire it.

Same here please fix this bug urgently.
