Dancing Dervish Hollow Palm

I'm playing a Hollow Palm Frenzy of Onslaught character and have Dancing Dervish (DD) for the additional Rampage it provides. There seems to be some kind of bug where it just reappears in my hands as if the Rampage ended but it's still going so i can't reengage the DD as each attack refreshes the already going Rampage. This means all my dmg disappears as Hollow Palm turns off, and then it kills me as I can't sustain my leech. This first happened to me in Simulacrum twice and now in a Ultimatum, always when i'm already at max Rampage and surrounded by enemies which makes disengaging from enemies to reset Rampage nearly impossible.
And you can't swap weapon set because the other is empty and if it isn't empty you don't deal dmg... makes using DD a liability atm
