I dont have that on my mana flask (and in the images you can see my mana flask is 100% full)

" No... actually, your images demonstrate that it has been used. You have a Divine Mana Flask equipped. 8 / 42 charges get consumed on use. The thing is not going to abruptly look half-empty after being used. You're using a tiny portion of them. It's exactly as the image that I posted portrays.

Yea I was not using my flasks in those two screen shots but the last patch fixed the issue with this.

Both of your screenshots showed it missing charges. Your character could not survive using it even a single time with its previous affixes. It is extremely obvious what happened here.
