Beast companion/summon missing (PS5)

I was playing in the Black Chambers (boss fight), and after about 3 deaths my companion summons is nowhere to be found. I have more than enough spirit for my build, and nothing had changed that I can see, but he will not reappear.

The only thing that looks odd is that my spirit level over my mana globe shows full amount (171/171), as opposed to the remaing amount after running all socketed spirit gems--as if all spirit gems are turned off, even though they are all on.

I've tried unsocketing and resocketing the gem(s), but that doesn't help. Tried other maps, and nothing.

Any clue as to what is causing this??
